- Any other concerning incident taking place in student housing that affects safety, well-being, or the residential environment -

- Any non‑emergency accident that needs to be documented for Campus Safety
- Injuries, work‑related exposures, property damage, or near‑miss events
All employees and students should immediately report any crime, suspicious circumstance or person, or emergency to Campus Safety by calling 360-596-5299 or 911. -
Bias Incident Reporting Form
Bias Incident Reporting
Use this form to report:
- Any bias‑motivated incident in which someone is targeted based on characteristics such as race, gender, religion, sexual orientation, ethnicity, veteran status, or disability
- Harassment, verbal slurs, or derogatory comments that are motivated by bias
- Graffiti, symbols, or written messages that target individuals or groups
- Incidents that may constitute hate crimes, including threats, intimidation, or property damage motivated by bias
- Any behavior that contributes to a hostile or unwelcoming environment
- Early signs of bias‑related behavior so the Behavioral Intervention Team can intervene, raise awareness, and support those affected -

Use this form to report:
- Any instance of harassment, including unwelcome conduct that creates a hostile, intimidating, or offensive environment
- Sexual harassment, such as unwanted sexual advances, requests for sexual favors, or other verbal/physical conduct of a sexual nature -
Apartment Incident Report

- Parking issues occurring at student housing locations, including improper parking, blocked spaces, or parking lot concerns
- Noise complaints, including loud or disruptive activity affecting the living environment
- Roommate issues, such as conflicts, disruptive behavior, or concerns impacting student well-being
- Lost keys and maintenance needs
- General questions, concerns, or non‑emergency incidents related to student housing -
